Job Overview
We are seeking a dedicated and compassionate Certified Social Worker (CSW) to join our dynamic healthcare and community support team. Responsibilities
- Conduct thorough mental health assessments and diagnostic evaluations utilizing clinical documentation standards and medical terminology to develop individualized treatment plans.
- Deliver evidence-based mental health interventions such as cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, and crisis intervention tailored to each client's needs.
- Provide case management services including intake procedures, patient assessment, and coordination of home & community care resources for clients with developmental disabilities, autism spectrum disorder, or substance use disorders.
- Facilitate individual and group therapy sessions focused on mental health counseling, addiction counseling, PTSD care, and social work interventions while maintaining adherence to HIPAA regulations and clinical confidentiality policies.
- Utilize EMR/EHR systems for accurate documentation of medical records, clinical notes, patient progress reports, and research data collection aligned with ICD-10/ICD-9 coding standards.
- Collaborate with multidisciplinary teams—including medical professionals specializing in geriatrics, pediatrics, special education, or psychiatric patient care—to optimize client outcomes.
- Engage in ongoing professional development through training in psychopharmacology, applied behavior analysis (ABA), crisis management, and cultural competence to enhance service quality.
Experience
- Proven experience working with children and adolescents in clinical or community settings; familiarity with child & family counseling preferred.
- Demonstrated expertise in mental health assessments, psychiatric patient care, and behavioral health interventions across diverse populations.
- Prior experience utilizing EMR/EHR systems for documentation; knowledge of CPT coding and ICD coding is advantageous.
- Background in home & community care environments; experience working with individuals with disabilities or developmental disorders such as autism spectrum disorder is highly valued.
- Strong skills in crisis management, trauma-informed care including PTSD treatment strategies, and addiction counseling techniques.
- Experience working within hospital settings or clinical research environments is a plus; familiarity with medical terminology and medical records management is essential.
- Ability to work effectively with clients from diverse cultural backgrounds while maintaining high standards of clinical confidentiality and ethical practice.
