Develop, implement, and evaluate individualized physical therapy treatment plans in accordance with long-term care regulations and best practices.
Perform comprehensive patient evaluations, including assessments of strength, mobility, balance, and functional abilities.
Administer therapeutic interventions to restore function, reduce pain, and prevent further disability.
Collaborate with physicians, nursing staff, administration, and interdisciplinary team members to coordinate comprehensive care.
Maintain high clinical standards and ensure compliance with federal, state, and facility guidelines.
Support the Physical Therapy department as needed, including collaboration on staffing and patient scheduling.
Provide mentorship, guidance, and support to new or less experienced team members when applicable.
Accurately document evaluations, treatment plans, progress notes, and discharge summaries in a timely manner.
Bachelor’s Degree in Physical Therapy from an accredited institution (Master’s or Doctorate preferred).
Active and unrestricted Physical Therapy license in the state of Florida.
Prior experience in a skilled nursing facility, hospital, or related healthcare setting required.
Strong clinical assessment and communication skills.
Ability to work effectively in a collaborative, team-oriented environment.
