We are seeking a dedicated Service Coordinator to support individuals with developmental disabilities through New Mexico’s DDSD Waiver program. This role focuses on coordinating care for clients and their families, ensuring health and safety, and collaborating with interdisciplinary teams and service providers.
Responsibilities include conducting monthly and quarterly in-person meetings, monitoring service effectiveness, maintaining compliance with DDSD standards, and completing timely documentation and reporting. You will advocate for clients, ensure meaningful day and person-centered practices, facilitate incident reporting and provider accountability.
Ideal candidates are able to stay on top of deadlines, compassionate, and skilled in communication, problem-solving, and teamwork. Knowledge or experience with DD Waiver services or a related field is highly preferred.
This is a full-time, non-exempt position requiring local travel and occasional on-call duties. A valid driver’s license, reliable transportation, and required state clearances/training are necessary.
