Licensed Physical Therapist — Part-Time or Full-Time
Senior-Focused Outpatient Program | Contra Costa County, CA
Are you a Physical Therapist who believes aging should come with strength, independence, and a secreat joy of aging through finess — not decline?
I'm looking for a passionate, collaborative PT to join me in growing an onsite outpatient physical therapy and fitness program inside a vibrant assisted living community. This isn't a typical clinic job. You'll work directly with residents in both our on-site gym and in-room settings, building real relationships and seeing the impact of your care every single day.
What You'll Be Doing
- Delivering personalized balance, strength, and mobility programs designed to keep residents active, confident, and out of the hospital
- Conducting in-home safety assessments and helping residents navigate their living spaces safely
- Training residents on assistive and electric mobility devices for safe, independent movement
- Collaborating closely with community leadership, nursing, and activities staff as a trusted clinical partner
- Delivering private fitness lessons to community residents outside of insurance-based care
- Managing a caseload using Spry EMR with AI features for optimizing time for our Medicare Part B and insurance patients
What We're Looking For
- Active PT license in California
- Genuine passion for geriatric care and a belief in the dignity and potential of older adults
- Strong communication skills — comfortable working closely with facility leadership, families, and interdisciplinary staff and supervising assistants and aides
- Self-motivated and comfortable working with autonomy in a community-based setting (this isn't a big hospital system — it's hands-on, relationship-driven care)
- Familiarity with Medicare Part B billing and documentation is a plus (or willingness to learn — we'll support you!)
- Curiosity or familiarity with newest AI tools and technologies — we're always looking for smarter ways to communicate, document, and design strategies that improve resident outcomes
