Ever Well Health Systems is seeking an organized, compassionate, team player to join their Residential Solutions team as a Residential Admissions Coordinator. The Residential Admissions Coordinator communicates with case managers, conservators, and other placement agencies regarding potential placements. The incumbent ensures the basic resident information data set has been received and is accurately entered into the data base and supports designation of appropriate follow up. Upon approved admission, assist with coordination of admission date with appropriate party. Prior to resident arrival, prepare admission packet for new resident, ensuring all information is complete and correct, and that all licensing requirements have been met. Provide update to staff to arrange for admittance of clients. After admission, ensure that all documents have been sent to the appropriate party and that resident file is ready for inspection, per licensing audit requirements.
DUTIES AND RESPONSIBILITIES
Acquire initial referral documentation and ensure data has been entered into database for review to determine appropriate plan of action.
Support coordination of admissions processes and procedures from initial referral stage, to after resident admittance.
Answer admissions related questions from the public related to the process, requirements, and procedures, maintaining excellent front-of-desk relations with customers and residents.
Assists in the marketing, promotion, and contact of county representatives for potential referral and placement.
Ensures that CCR Title XXII, regulatory agency and facility requirements, including initial and ongoing resident documentation standards are maintained.?
Assist with arranging transportation for admissions.
Provide data for periodic admissions reports producing key performance indicators.
Participates in and facility meetings and training as assigned, requested, or required.
Performs other duties as assigned.
