The Primary Care Nurse Practitioner provides direct care and education to Health Center patients and ensures continuity and quality of care. Practitioner will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.
Responsibilities:

Provide direct patient care, counseling, and teaching to our primary care population in an ambulatory setting. Care for both acute and chronic conditions for new and established patients.

Perform complete histories and physical examinations.

Order, interpret, and evaluate appropriate laboratory and diagnostic tests.

Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.

Order medications and write prescriptions according to organizational and regulatory policies and procedures, including agreement with collaborating physician as required

Collaborate and consult with a multidisciplinary team of providers and other staff, including supervising physician, as appropriate.

Demonstrate expert clinical judgment skills to function in an independent role.

Graduate of Master's of Science in Nurse Practitioner program or have completed a Certified Nurse Practitioner program as well as Massachusetts licensure and Board Certification.

Experience with Patient Centered Medical Home model and working with underserved populations

Knowledge of and experience with EMR and Microsoft Office systems required, with Epic strongly preferred
