We are seeking a Physical Therapist for a 13-week assignment in a skilled nursing and long-term care setting in San Antonio, Texas. This role is ideal for a therapy professional who is confident in evaluating patients, developing treatment plans, and supporting functional recovery for residents.
Responsibilities
Evaluate patients and develop individualized therapy plans
Provide skilled physical therapy services in a SNF/LTC setting
Support mobility, strength, balance, and functional improvement
Document treatment progress and patient response accurately
Collaborate with the care team to support patient outcomes
Maintain safe therapy practices and follow facility standards
Requirements
Active Texas Physical Therapist license required
Minimum 1 year of Physical Therapist experience required
Skilled Nursing Facility or Long-Term Care experience preferred
Strong clinical assessment and patient care skills
Ability to work independently and as part of a care team
