Job Title: Part-Time Permanent, Certified Nursing Assistant (CNA) – Home Health
Location: Colorado
Schedule: Tuesdays and Wednesdays with rotated weekends and holidays.
Reports To: Nursing Supervisor
Position Summary:
As a Part-Time Permanent CNA with Primary Home Health Care, Inc., you will play a vital role in delivering compassionate, hands-on care to individuals in the comfort of their homes. Under the supervision of a Registered Nurse, you will assist patients with daily living activities, therapeutic exercises, and health monitoring.
Your expertise in basic life support (BLS), EMR systems, and prior experience in long-term or critical care settings will be essential in improving the quality of life for our clients, especially those with developmental disabilities or memory care needs.
Key Responsibilities
- Therapeutic Assistance: Perform basic therapeutic procedures under therapist direction, such as range-of-motion exercises and ambulation support.
- Personal Care Services: Assist with bathing, dressing, oral hygiene, nail care, shaving, and skin care.
- Client Monitoring: Observe and report changes in the client’s condition to the RN and document care per agency protocol.
- Safety & Infection Control: Follow infection control procedures; report safety concerns or hazards to the Case Manager or Supervisor.
- Team Communication: Maintain clear communication with all interdisciplinary team members; participate in care team meetings.
- Confidentiality: Protect client information per HIPAA regulations and agency privacy policies.
Job Qualifications:
Required:
- Must be available Tuesdays and Wednesdays
- Active CNA license in the State of Colorado
- CPR/BLS certification
- At least 21 years of age
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to pass a background check, drug screening, and physical exam
- Must be available Tuesdays and Wednesdays.
- Must do weekends and holidays on rotation (once a month minimum)
Preferred:
- Experience working with developmental disabilities or memory care patients
- Proficiency in Electronic Medical Record (EMR) systems
- Minimum 6 months of supervised CNA experience in a healthcare setting
- Must do weekend rotations, both Saturday & Sunday (minimum of once a month)
