Engage in pharmacotherapy consults, offering expertise in medication therapy management, optimizing therapeutic regimens, and addressing medication-related concerns.
Contribute to the development and education of pharmacy staff, enhancing their knowledge on current drug therapies, policies, and clinical practices to improve patient care.
Regularly assess patient medication use, reviewing orders and clinical data to ensure appropriate treatment, dosing, and safety for diverse patient populations.
Accurately process medication orders, including verifying correct dosages, patient specifics, and drug interactions, and addressing any discrepancies or concerns.
Perform the final check of all medications to ensure accuracy in patient-specific orders, including proper storage, restocking, and ensuring the safety of sterile and nonsterile compounded medications.
Assess and address the age-specific needs of patients, ensuring medication therapies are appropriate and adjusted for pediatric, adult, and geriatric populations.
Ensure all pharmaceutical practices are in full compliance with local, state, and federal laws and regulations, including adherence to accreditation standards and professional guidelines.
Work closely with physicians, nurses, and other healthcare providers to ensure effective medication management, providing recommendations and clarifications as needed to optimize patient outcomes.
