Physical Therapist (PT) – Skilled Nursing Facility
Location: Port Charlotte, FL
We’re currently seeking a Physical Therapist (PT) to join a well-run Skilled Nursing Facility in Port Charlotte, FL. This is a full-time, permanent opportunity for a therapist who is comfortable working in a SNF / long-term care setting and enjoys treating a medically complex, geriatric population.
Position Overview
The Physical Therapist will evaluate residents, develop individualized treatment plans, and deliver high-quality rehab services aimed at improving mobility, strength, balance, and overall functional independence.
Key Responsibilities

Perform physical therapy evaluations and establish appropriate treatment plans

Provide skilled therapy services to short-term rehab and long-term care residents

Collaborate with interdisciplinary teams including nursing, OT, SLP, and physicians

Document treatments and progress in accordance with facility and regulatory standards

Educate residents and families on treatment goals and discharge planning

Maintain compliance with Medicare and SNF documentation requirements
Qualifications

Active Florida Physical Therapist license

Experience in a Skilled Nursing Facility or Long-Term Care setting is a plus

Strong clinical judgment and time-management skills

Ability to work independently while collaborating with a team

Experience with geriatric and post-acute rehab populations preferred
