Social Services Coordinator – Job Description
Position Summary: Social Services Coordinator is responsible for assisting patients with access to social, financial, and community resources. This role ensures that patients and their families are connected to appropriate programs such as Medicaid, SNAP, QMB, SSI, and other supportive services. The coordinator serves as a liaison between patients, healthcare providers, government agencies, and community organizations to promote continuity of care and enhance patient well-being.
Key Responsibilities
Patient Support & Case Management
- Conduct screenings to identify patients’ eligibility for benefits (Medicaid, QMB, SNAP, LIS, SSI, etc.).
- Assist with completion and submission of applications, renewals, and required documentation.
- Maintain patient confidentiality and ensure compliance with HIPAA and organizational policies.
- Follow up with patients to confirm application status and address pending requirements.
Community & Resource Coordination
- Provide information about local, state, and federal resources, including housing, transportation, food programs, and financial assistance.
- Refer patients to external agencies or community partners as needed.
- Build and maintain strong working relationships with government agencies (e.g., DCF, Social Security, USCIS) and community resource providers.
Administrative & Compliance
- Maintain accurate and timely case notes, logs, and reports in designated systems.
- Track applications and outcomes to ensure follow-up and accountability.
- Assist with departmental reporting and data collection as requested by management.
Team Collaboration
- Work closely with Social Services team members, medical staff, and administrators to coordinate patient support.
• Participate in departmental meetings, trainings, and professional development activities. • Provide coverage and support to other coordinators as needed across multiple centers.
Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in Social Work, Human Services, or related field preferred.
- Minimum 1–2 years of experience in social services, healthcare access, or community resources coordination.
- Knowledge of Medicaid, SNAP, SSI, and related programs strongly preferred.
- Bilingual (English/Spanish or English/Creole) highly preferred.
- Strong organizational, communication, and interpersonal skills.
- Proficient in Microsoft Office and comfortable learning new software systems.
