Lead Patient Advocate at Usher Care
Getting older with a health condition means managing a lot: multiple doctors who do not talk to each other, appointments, medications, paperwork. Most Medicare enrollees and their families handle it alone. Usher Care pairs them with someone who knows the system and guides them through it. You will do that work yourself and build the team that does it at scale.
This is a full-time, work-from-home, salaried W2 role. You will carry a panel of your own clients, lead a group of part-time Patient Advocates, and work directly with our supervising physician and founders. This is an early role, so you will help decide how the work gets done, not just execute it.
What you'll do
- Serve manageable patient advocacy panel: help patients understand doctors’ guidance, prepare for appointments, coordinate providers, find local resources, and follow through on care plans
- Lead a team of Patient Advocates: onboarding, coaching, weekly check-ins, caseload assignment, and being the first call when something gets complicated
- Own quality: review documentation, spot patterns across clients, and make sure our standards hold up in practice
- Escalate clinical questions to the supervising physician and teach advocates when to escalate
- Help build the training program, playbooks, and escalation protocols as we grow
- Interview and help select new advocates
Who we're looking for
- Several years in a patient-facing health care or social work role, with fluency in how the system works: care coordination, case management, discharge planning, home care, social work, community health, or clinical practice. Medical Assistant, LPN, LCSW, and other backgrounds are welcome.
- Experience leading, training, or mentoring people, formally or informally. You do not need a manager title
- Comfort and patience with seniors and their adult children, including when a family is frightened or upset
- Community health worker certification a plus
- Judgment about what you can handle and what needs a clinician
- Comfortable working without a playbook and then writing one
- Reliable internet and a private space at home for confidential conversations
Terms
- Part-time to start then growing to full-time
- $35 to $45 / hour W2 employee
- Ongoing collaboration with a licensed supervising physician
- Remote / work from home
- Early-stage role with meaningful influence on how the care model develops
Pay: $35.00 - $45.00 per hour
