Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our hospice home healthcare team must have a year previous experience in hospice home care. The ideal candidate will be responsible for coordinating patient care, taking on call, conducting assessments, and ensuring that patients receive the necessary services to achieve optimal health outcomes. This role requires a strong background in both clinical and case management settings, with a focus on patient-centered care.
Duties
- Taking on call and Conduct comprehensive patient assessments to determine healthcare needs and develop individualized care plans.
- Coordinate patient care across various settings, including patients homes
- Perform diagnostic evaluations and monitor vital signs to assess patient conditions.
- Collaborate with interdisciplinary teams to ensure effective utilization management and continuity of care.
- Provide education and support to patients and their families regarding treatment plans and available resources.
- Advocate for patients’ needs within the healthcare system, ensuring access to appropriate services.
- Document all patient interactions and maintain accurate records in compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance patient care delivery.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in case management or a related field is preferred.
- Strong clinical background in areas such as OR, clinic, hospital settings, or behavioral health.
- Proficient in conducting patient assessments and utilizing diagnostic evaluation techniques.
- Excellent communication skills, both verbal and written, with the ability to work collaboratively within a team.
- Knowledge of utilization management principles and practices.
- Ability to manage multiple priorities while maintaining attention to detail.
