RN (Registered Nurse) – Case Management
Location: Bronx, NY
Duration: 13 Weeks
Shift: Days | Monday–Friday, 8:00 AM–4:30 PM
Hours: 37.5 Hours/Week

Job Responsibilities
- Provide inpatient case management and discharge planning services.
- Develop and coordinate discharge plans for patients transitioning to home, community settings, or skilled nursing facilities (SNF).
- Make appropriate referrals and coordinate resources to support safe patient discharge.
- Work collaboratively with physicians, social workers, patients, families, and other members of the interdisciplinary team.
- Round on assigned units with physicians and the healthcare team.
- Assist with obtaining housing resources, shelter packets, hemodialysis packets, wound VAC referrals, and other discharge-related services.
- Manage an assigned caseload, typically ranging from 17–25 patients, while maintaining effective time management and prioritization.
- Function independently while supporting patient-centered discharge planning.
- Document case management activities accurately in Allscripts.
Requirements
- Active New York RN License.
- BSN required.
- Current BLS certification.
- Minimum 1 year of inpatient case management experience.
- Experience with discharge planning in an inpatient setting.
- Ability to manage a caseload of approximately 20+ patients.
- Strong critical thinking, communication, collaboration, and time management skills.
- Ability to work independently and as part of an interdisciplinary team.
- PRI and SCREEN certifications required; certifications may be completed online.
- Must be comfortable working primarily with Med-Surg and Telemetry patient populations.
- Allscripts experience preferred.
Preferred Qualifications
- Prior critical care experience.
- Experience with CarePort, MCG, InterQual, and/or similar case management criteria.
- Previous Utilization Review experience.
- Experience coordinating complex discharge needs and community resources.
- Strong knowledge of inpatient discharge planning and referral processes.
Pay: $65.00 - $72.00 per hour
