Job Summary
We are seeking a compassionate and dedicated Certified Home Health Aide to join our team in providing exceptional in-home care to individuals with diverse needs. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately
- Administer medications and manage medical documentation in compliance with home health regulations and HIPAA standards
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques
- Support patients with catheter care, wound management, hospice care, and end-of-life support as needed
- Operate Electronic Health Record (EHR) systems to document patient progress and medical records efficiently
- Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment using proper lifting techniques like Hoyer lifts when necessary
Qualifications
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in accordance with state requirements
- Proven experience working in home healthcare settings or assisted living environments
- Knowledge of medical terminology and procedures related to geriatrics, disabilities, and chronic conditions
- Experience with EMR/EHR systems for accurate documentation of patient care activities
- Ability to work independently while following detailed care plans and protocols
- Strong communication skills to effectively interact with patients, families, and healthcare teams
- Physical stamina for heavy lifting, transferring patients using assistive devices like Hoyer lifts, and providing hands-on care
- Familiarity with home & community care regulations including HIPAA compliance and patient privacy standards

Pay: $18.00 - $20.00 per hour
