Littleton Care & Rehabilitation Center is seeking a compassionate, energetic, and experienced Social Services Director to join our leadership team. We are a 35-bed skilled nursing facility dedicated to providing exceptional care and support to residents with complex medical and rehabilitative needs. Our mission is to provide outstanding care while helping residents achieve their highest level of independence and quality of life.
This position is ideal for a social work professional who is passionate about resident advocacy, mental health, education, problem-solving, and interdisciplinary collaboration. Due to our active rehabilitation program and high skilled census, the ideal candidate must be highly organized and experienced in discharge planning, care coordination, and transitions of care.
Qualifications
- Bachelor's Degree in Social Work (BSW) or related social science field required.
- Minimum of two years of Director or leadership experience preferred.
- Skilled nursing, rehabilitation, long-term care, or healthcare experience preferred.
- Knowledge of Medicare, Medicaid, resident rights, and federal and state regulations.
- Strong communication, organizational, and problem-solving skills.
- Ability to work effectively with residents, families, physicians, hospitals, and interdisciplinary team members.
Essential ResponsibilitiesResident Care & Social Services
- Assess and identify the medical-related social and emotional needs of residents.
- Develop and implement individualized care plans in collaboration with residents, families, physicians, and the interdisciplinary team.
- Provide counseling, support, and advocacy for residents and families.
- Assist residents with financial, legal, and healthcare decision-making needs.
- Address resident concerns, grievances, and quality-of-life issues.
Discharge Planning & Care Coordination
- Coordinate comprehensive discharge planning services from admission through discharge.
- Collaborate with hospitals, physicians, therapy teams, community agencies, and family members to ensure safe and successful transitions of care.
- Arrange referrals to home health, hospice, assisted living, community resources, and other post-discharge services as appropriate.
- Ensure discharge plans meet regulatory requirements and support positive resident outcomes.
- Consult with physicians and healthcare team members regarding psychosocial factors relating to residents' illnesses, treatments, and discharge plans.
Leadership & Compliance
- Directly supervise social services staff.
- Interview, hire, train, mentor, and evaluate employees.
- Plan, assign, and direct work activities.
- Conduct performance evaluations and address employee concerns.
- Develop, implement, and maintain social services policies and procedures.
- Ensure compliance with federal, state, Medicare, and Medicaid regulations.
- Orient and train employees on social services policies and procedures.
- Monitor departmental supplies, equipment, and budget needs.
