Telos Health Systems is seeking a dedicated and experienced Nurse Practitioner (NP) to provide high-quality primary and internal medicine care to residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). This position is ideal for a provider who values autonomy, collaboration, and compassionate care for the geriatric population.

- Serve as the primary in-house medical provider within assigned facilities
- Manage care for both short-term rehabilitation and long-term care residents
- Conduct comprehensive assessments, physical exams, and diagnoses
- Evaluate and manage acute and chronic medical conditions
- Prescribe medications and treatment plans as appropriate
- Complete monthly wellness and stability visits
- Assess and respond to changes in patient condition
- Participate in Advanced Care Planning discussions
- Collaborate with interdisciplinary teams, attending physicians, and medical directors
- Maintain accurate documentation and CPT coding
- Coordinate referrals to specialists and additional healthcare services as needed
Required
- Active and unrestricted APN/APRN license
- Current DEA registration
- Master’s Degree in Nursing
- Strong clinical judgment and communication skills
- Ability to thrive in a fast-paced healthcare environment
Preferred
- Minimum of one year of experience in Skilled Nursing, long-term care, or post-acute settings
- Experience working with geriatric patients
