Responsibilities:
Provide high-quality, patient-centered care to our long-term care residents, including administering medications, wound care, and IV therapy
Develop and implement individualized care plans in collaboration with the healthcare team
Perform routine vital signs, monitor and report changes in resident condition to the healthcare team
Participate in resident assessments, care plan development, and care coordination
Collaborate with interdisciplinary teams to ensure seamless transitions and optimal resident outcomes
Maintain accurate and thorough records of resident care
Actively participate in quality improvement initiatives to enhance the resident experience

Requirements:
Current RN licensure in the state of Illinois
