JOB SUMMARY:
The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.
This position reports to the Director of Rehabilitation Services.
JOB DUTIES:
Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
