Experienced Hospice or Home Health Sales Professional — Join 365 Hospice
365 Hospice is growing, and we are looking for an experienced Hospice Clinical & Community Liaison to join our team in the Pittsburgh area.
This position is ideal for an established hospice or home health sales professional who understands healthcare referral relationships, knows how to develop a territory, and has a proven track record of generating referrals and admissions.
We are looking for someone who already understands the healthcare sales environment and can hit the ground running.
THE ROLE
As a Hospice Clinical & Community Liaison, you will be responsible for developing and maintaining referral relationships throughout your assigned territory.
You will work directly with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, social workers, case managers, discharge planners, and other healthcare professionals to educate them about 365 Hospice and identify appropriate hospice patients.
This is a relationship-driven healthcare sales position.
The successful candidate will be comfortable working independently, developing a territory, identifying referral opportunities, and consistently growing hospice admissions.
RESPONSIBILITIES
- Develop and grow hospice referral relationships within an assigned territory
- Establish and maintain relationships with physicians, hospitals, SNFs, assisted living facilities, home health agencies, and other healthcare organizations
- Generate qualified hospice referrals and help drive census growth
- Conduct regular face-to-face visits with referral sources
- Develop and execute a strategic territory sales plan
- Identify new referral sources and opportunities for growth
- Maintain and grow existing referral relationships
- Educate healthcare professionals regarding hospice eligibility, services, and benefits
- Conduct presentations and educational programs for healthcare professionals and community organizations
- Work closely with the intake, clinical, and leadership teams to ensure timely follow-up on referrals
- Monitor referral activity, admissions, and territory performance
- Maintain accurate records of sales activity and referral sources
- Participate in community and professional events representing 365 Hospice
- Develop a strong understanding of the competitive hospice and home health market
- Become a trusted resource for healthcare professionals and families throughout the territory
REQUIRED EXPERIENCE
Previous hospice or home health sales experience is REQUIRED.
Candidates must have demonstrated experience in healthcare sales, business development, marketing, or community liaison work specifically within the hospice or home health industry.

QUALIFICATIONS
- 1+ years of hospice or home health sales/business development experience preferred
- Proven history of generating healthcare referrals
- Existing relationships within the Pittsburgh healthcare community strongly preferred
- Strong understanding of hospice and/or home health services
- Excellent communication and presentation skills
- Strong relationship-building abilities
- Highly motivated and self-directed
- Organized and capable of managing a territory independently
- Professional appearance and excellent interpersonal skills
- Valid driver's license and reliable transportation
Candidates without hospice or home health sales experience will not be considered.

COMPENSATION
$65,000–$90,000 Base Salary + Uncapped Commission/Bonus
Compensation will be based on experience, existing referral relationships, and proven sales performance.
Target Total Compensation: $90,000–$120,000+
Top performers will have the opportunity to significantly exceed the target compensation through our incentive program.
