We are seeking a Licensed Cognitive Therapist to provide evidence-based cognitive rehabilitation and therapeutic intervention to individuals in their homes and community settings. The ideal candidate has experience supporting individuals with acquired brain injuries, cognitive impairments, IDD, or co-occurring behavioral and neurological conditions. This role involves assessing cognitive functioning, developing individualized treatment plans, and collaborating with an interdisciplinary team to maximize each individual's functional independence and quality of life.
- Conduct comprehensive cognitive assessments to evaluate memory, attention, executive function, processing speed, and functional cognition in the home setting

Develop and implement individualized cognitive rehabilitation treatment plans based on assessment findings and individual goals

Provide direct cognitive therapy interventions including compensatory strategy training, memory rehabilitation, attention training, and executive function skill building

Collaborate with physicians, nursing staff, occupational therapists, behavioral specialists, and DSPs to ensure integrated, person-centered care

Educate individuals, family members, caregivers, and DSPs on cognitive strategies, environmental modifications, and support techniques

Monitor and document individual progress, adjust treatment plans based on clinical response and data, and communicate outcomes to the interdisciplinary team

Complete all clinical documentation including evaluations, session notes, progress reports, and discharge summaries accurately and within required timeframes

Ensure all services are delivered in compliance with Title 28 home health regulations and applicable waiver program standards

Participate in ISP meetings, care conferences, and QAPI initiatives as needed
- Current, unrestricted Pennsylvania licensure as a cognitive therapist or in a related clinical discipline (psychology, counseling, neuropsychology, or speech-language pathology with cognitive rehabilitation focus)

Master's degree or higher in a relevant clinical field

Minimum 1–2 years of experience providing cognitive therapy or rehabilitation services in a home health, outpatient, or community-based setting

Strong knowledge of cognitive assessment tools and evidence-based cognitive rehabilitation frameworks

Valid Pennsylvania driver's license and reliable personal transportation

Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting

Current CPR/BLS certification

Strong clinical documentation and communication skills
- Experience working with individuals with IDD, acquired brain injury (ABI), stroke, dementia, or traumatic brain injury (TBI)

Familiarity with Pennsylvania ODP or OLTL/Community HealthChoices waiver programs

Experience with Axxess or Therap EHR platforms (a plus)

Knowledge of Title 55 (Chapters 6100/6400) residential habilitation framework
