Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record patients’ vital signs and health status to ensure optimal care
- following prescribed care plans while maintaining HIPAA confidentiality
- Support clients with meal preparation, cooking nutritious meals tailored to dietary needs
- Conduct patient observation to identify changes in condition or behavior and communicate findings promptly
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and laundry
- Follow established care plans and collaborate with healthcare professionals to deliver consistent, high-quality care
- Provide emotional support and companionship to foster a positive living environment
- Adhere to all safety protocols including CPR and First Aid certifications to respond effectively in emergenciesExperience
- Prior experience in home health, assisted living, nursing home, or group home settings preferred
- Demonstrated ability working with individuals with developmental disabilities, autism, dementia, or Alzheimer’s care needs
- Knowledge of patient monitoring techniques and vital sign measurement required
- Experience assisting with medication administration and managing care plans preferred
- Familiarity with HIPAA regulations ensuring privacy and confidentiality of patient information
- Strong communication skills for working effectively with clients, families, and healthcare teams l
