Job Summary
Life Care Center of Elkhorn/Omaha is seeking a dedicated and skilled Physical Therapist (PT) to join our healthcare team. The ideal candidate will possess a strong background in physiotherapy with expertise in outpatient and inpatient settings, including acute care, geriatrics, pediatrics, neurology, and orthopedics. As a Physical Therapist, you will develop individualized care plans, deliver effective therapy interventions, and promote optimal recovery and mobility for diverse patient populations.
Duties
- Assess patients’ physical conditions through comprehensive evaluations to determine appropriate treatment strategies.
- Develop and implement personalized care plans that align with clinical standards and patient goals.
- Administer therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to improve mobility and reduce pain.
- Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless patient transitions across care settings.
- Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR) management protocols.
- Provide education to patients and their families on injury prevention, rehabilitation exercises, and health maintenance strategies.
- Work within outpatient clinics, inpatient units, home health environments, or rehabilitation centers to deliver high-quality patient care.
- Stay current with physiotherapy best practices, physiology knowledge, medical terminology, and clinical documentation standards.
Experience
Candidates should have prior experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings. Experience working with geriatric patients, pediatric populations, neurology cases, orthopedics, or home & community care is highly desirable. Familiarity with EMR systems and electronic health records (EHR) management is essential. Additional qualifications include:
- Demonstrated expertise in discharge planning and care plan development for diverse patient needs.
- Proven track record working in acute care environments with inpatient experience preferred.
- Knowledge of physiology, anatomy, trigger point therapy techniques, and outpatient rehabilitation procedures.
- Experience managing clinical documentation standards accurately while adhering to healthcare regulations.
- Ability to collaborate effectively within multidisciplinary teams to optimize patient outcomes across hospital, outpatient clinic, home health, or outpatient rehabilitation settings.
