Requirements
- Active and unrestricted Maryland Registered Nurse (RN) license required
- Minimum 2 years of Home Health nursing experience preferred
- Previous Case Management experience preferred
- OASIS experience required or preferred (depending on client requirements)
- Current BLS/CPR certification required
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the Baltimore service area
- Strong assessment, critical thinking, and care coordination skills
- Excellent communication and documentation skills
- Ability to pass background screening and onboarding requirements
Responsibilities
- Complete comprehensive nursing assessments for patients of all ages in the home setting
- Develop, implement, and update individualized plans of care
- Coordinate patient services with physicians, therapists, caregivers, and interdisciplinary healthcare teams
- Complete OASIS assessments and maintain accurate clinical documentation
- Monitor patient progress and adjust care plans as appropriate
- Educate patients and caregivers regarding diagnoses, medications, treatments, and disease management
- Ensure compliance with Medicare Conditions of Participation and agency policies
- Participate in case conferences and quality improvement initiatives
- Maintain timely documentation and communication with referral sources and physicians
- Deliver compassionate, patient-centered care while promoting independence and positive health outcomes
