Job Overview
An acute care hospital is seeking an experienced Registered Nurse (RN) Case Manager to join its Medical Management Department. The RN Case Manager will provide comprehensive care coordination, discharge planning, and psychosocial support services for patients and families while collaborating with multidisciplinary teams to ensure safe and timely transitions of care.
Responsibilities
- Complete initial and concurrent insurance reviews.
- Perform PRI assessments for patients requiring skilled nursing facility placement.
- Conduct psychosocial assessments through patient/family interviews, chart reviews, and interdisciplinary collaboration.
- Develop, implement, and monitor effective discharge and treatment plans.
- Coordinate care with patients, families, providers, and healthcare teams to ensure continuity of care.
- Provide patient advocacy, education, and support throughout the care process.
- Participate in patient-focused conferences and multidisciplinary meetings.
- Utilize leadership skills in discharge planning, conflict resolution, team collaboration, and staff education.
- Participate in quality improvement programs, reporting activities, staff meetings, and continuing education.
- Demonstrate knowledge of ethical and legal issues related to end-of-life care, informed consent, advance directives, DNR orders, and patient rights.
- Identify and assist with appropriate interventions for cases involving abuse, neglect, violence, or safety concerns.
- Prioritize workload effectively based on patient needs, medical plans, and departmental standards.
- Utilize advocacy, mediation, and negotiation skills with patients, families, healthcare teams, and community resources.
Qualifications
- New York State RN license required.
- Minimum 2 years of recent acute case manager experience.
- AHA - BLS
