Job Summary
We are seeking a dedicated and compassionate Social Worker specializing in Skilled Nursing to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients in post-acute, inpatient, outpatient, and long-term care settings. Duties
- Conduct thorough patient assessments, including psychosocial evaluations and diagnostic evaluations, to inform individualized care plans.
- Provide clinical counseling, individual and group therapy sessions, and behavioral health interventions tailored to diverse patient populations such as seniors, children, or individuals with disabilities.
- Coordinate discharge planning by collaborating with healthcare teams, families, and community resources to ensure seamless transitions from hospital or nursing home settings.
- Offer crisis intervention and motivational interviewing to support patients facing mental health challenges or end-of-life decisions.
- Maintain accurate medical documentation adhering to HIPAA regulations and social work practice standards; utilize EMR systems for efficient record keeping.
- Engage in case management activities including data collection, treatment planning, and follow-up to optimize patient outcomes.
- Support specialized care initiatives such as hospice & palliative medicine, Alzheimer's care, PTSD care, autism spectrum disorder interventions, and working with individuals with developmental disabilities.
Requirements
- Minimum of Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master’s Degree preferred.
- Proven experience in hospital settings, nursing homes, assisted living facilities, or outpatient clinics with familiarity in inpatient experience and post-acute care.
- Current Social Work license in the state preferred
- Knowledge of regulatory standards and compliance requirements.
- Working knowledge of personal computer and software applications used in job functions, (word processing, graphics, databases, spreadsheets, etc.)
- Strong organizational and analytical skills; oral and written communication skills.
- Working knowledge and ability to apply professional standards of social work practice in job situations.
- Current knowledge of state and federal laws and regulations that apply to practice of social work in long-term care.

Pay: $55,000.00 - $70,000.00 per year
