Make a Difference Through One-on-One Care
Community Horizons works with individuals who live in their own homes and choose, hire, and direct their own caregivers. This gives members the opportunity to build a caregiving team that fits their individual needs, routines, and lifestyle.
We are currently helping a young adult male find a dependable and compassionate caregiver to provide one-on-one support in his home while his mom is at work.
Schedule
7:00 AM–3:00 PM and possibly other hours as needed or wanted
He requires 24-hour support, and this position would be part of the caregiving team that helps ensure he has reliable care throughout the day.
Caregiver Responsibilities
Responsibilities may include:
- Assisting with daily routines and personal cares as needed
- Assisting with pivot transfers
- Providing support related to his trach and established care routine
- Assisting or providing support with his self-catheterization routine as needed
- Providing supervision, companionship, and one-on-one support
- Encouraging independence and allowing him to do as much for himself as possible
- Helping him get out into the community and enjoy activities when desired
- Maintaining a safe, comfortable, and respectful environment
Transportation
He has his own wheelchair-accessible van, making it possible to get out of the house for activities, appointments, errands, or just a change of scenery.
A valid driver’s license is strongly preferred so caregivers have the ability to drive his accessible van when he would like to go somewhere.
Who We’re Looking For
The right caregiver is someone who is:
- Dependable and consistent
- Patient, caring, and respectful
- Comfortable providing one-on-one care
- Willing to learn his individual routines and care needs
- Comfortable working with someone who has a trach and uses a wheelchair
- Focused on supporting independence, choice, and quality of life
- Able to build a trusting relationship with both the member and his family
- Ideally has a valid driver’s license
Experience is helpful, but finding the right person who is dependable, willing to learn, and genuinely cares is just as important.
This is more than simply completing caregiving tasks. It’s an opportunity to support a young adult in maintaining his independence, staying active, and living his life on his own terms while receiving the assistance he needs.
Pay: From $17.00 per hour
