Licensed Vocational Nurse (LVN) – Full-Time (HCS Program)
Adaptive Aids, LLC is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care and support within our Home and Community-based Services (HCS) program. This role is ideal for a dependable, detail-oriented nurse who is passionate about delivering quality care in community settings.
Position Overview
Under the supervision of a Registered Nurse, the LVN provides direct patient care, medication administration, documentation, and coordination of services to support individualized care plans and ensure compliance with HCS and state regulations.
Key Responsibilities
Client Care & Nursing Services
- Provide nursing care in a manner that promotes client safety and comfort
- Administer medications and treatments (e.g., wound care, catheter care, vitals monitoring)
- Monitor and report changes in client condition to the RN and care team
- Assist in developing and implementing individualized care plans
Coordination & Support
- Coordinate services such as therapy appointments and follow-ups
- Communicate effectively with physicians, families, and interdisciplinary teams
- Support direct care staff in emergency situations and proper care procedures
Documentation & Compliance
- Complete accurate and timely documentation in electronic systems
- Ensure documentation meets regulatory, legal, and audit standards
- Maintain accuracy, timeliness, and completeness in all records
- Follow all infection control protocols and universal precautions
- Ensure compliance with HCS regulations, HHSC requirements, and company policies
Requirements
- Active Texas LVN license in good standing
- Current CPR/BLS certification
- Minimum 1 year of nursing experience preferred
- HCS, IDD, or home health experience preferred
- Valid Texas Driver’s License or State ID
- Ability to pass background and Nurse Aide Misconduct Registry checks
- Reliable transportation
- Strong communication and organizational skills

Role Expectations

- Support compliance with HCS program standards and regulations
Physical Requirements
- Ability to lift, transfer, and reposition clients as needed
- Ability to stand, walk, bend, and perform physical caregiving tasks for extended periods
- Ability to respond to emergency situations quickly and appropriately
Compensation & Schedule
- Salary: $45,000 – $60,000 per year (based on experience)
- Full-time position with consistent hours
- Flexible scheduling based on client needs
- Combination of on-site visits and limited remote work (as applicable)
