ABOUT THE ROLE
Consultant Services of MN is recruiting on behalf of two licensed Minnesota 245D providers, CarePro Home Health Care and Happy Living Options. Both are seeking a qualified Designated Coordinator (DC) and/or Designated Manager (DM) to support home and community-based services in the Minneapolis, St. Paul, and St. Cloud areas. Two positions are available. You will be told which agency you are being considered for at first contact.
RESPONSIBILITIES
Provide the supervision and oversight of direct support staff required of a Designated Coordinator or Designated Manager under 245D.
Develop, review, and revise Coordinated Service and Support Plan Addenda (CSSP-A) and monitor progress toward each person's outcomes.
Ensure service delivery matches each person's coordinated service and support plan and the agency's policies and procedures.
Oversee staff orientation, annual training, and documentation of required training.
Manage incident reporting, maltreatment reporting obligations, emergency use of manual restraint procedures where applicable, and internal review.
Maintain service recipient records, prepare for DHS licensing reviews, and respond to correction orders.
Coordinate with case managers, county and lead agency staff, families, and legal representatives.
Participate in quality assurance and self-monitoring activities.
REQUIRED QUALIFICATIONS
You must meet one of the qualification pathways under Minn. Stat. 245D.081. These pathways combine education (a bachelor's degree, associate degree, or diploma in a health or human services field, or documented training) with a corresponding amount of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older. The less education you hold, the more qualifying experience is required.
In addition, you must:
Be eligible under a DHS background study.
Have completed, or be able to complete before designation, the orientation and training required under 245D.
Have reliable transportation and a valid driver's license, with travel between sites in the Minneapolis, St. Paul, and St. Cloud areas.
Be able to produce documentation on request: transcripts, diplomas, training certificates, and verifiable employment history showing dates, hours, job duties, and supervisory responsibility.
HOW WE VERIFY
Every applicant's qualifications are independently verified before designation. That includes transcripts, degree conferral, qualifying direct-care and supervisory experience, completed training, and background study eligibility. DHS provides a verification form documenting how an individual meets the DC/DM requirements, and it will be completed for the person hired. If you cannot document the pathway you claim, you will not be designated, regardless of prior job titles.
