As a Physical Therapist (PT), you will evaluate residents, create individualized treatment plans, and deliver hands-on therapy to promote recovery and independence. This role offers the opportunity to work in a collaborative, interdisciplinary environment where your expertise directly impacts patient outcomes.
Degree in Physical Therapy from an accredited program
Active, unrestricted New York State Physical Therapist (PT) license
BLS certification required
Experience in sub-acute rehab or long-term care preferred (new graduates welcome)
Strong communication, patient education, and documentation skills
Perform comprehensive physical therapy evaluations
Develop and implement evidence-based treatment plans
Provide therapeutic exercises, mobility training, manual therapy, and restorative treatments
Evaluate, recommend, and assist with orthotic and prosthetic devices
Monitor patient progress and adjust treatment plans as needed
Educate residents and families on home exercise programs and recovery goals
Supervise Physical Therapy Assistants and collaborate with nursing and rehab staff
Maintain accurate and timely clinical documentation in compliance with regulations

$95,000 per year (based on experience)
$90,000 per year (new graduates welcome)
$2,000 Sign-On Bonus
