Under the direction of the Vice president of Quality, Standards and Compliance and Social Services, is official representative to all referral agencies.
DESCRIPTIN OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

Keep an on-going record of vacancies or proposed vacancies in order to share this information with referral agencies and administrative staff.

Be recipient of all telephone inquires related to intake and all official referrals. Give tours to interested families and Community Agencies.

Maintain all referral information on computer.

Assist in developing a marketing plan for residential services.

Coordinate the flow of referrals and follow up referrals to the appropriate professional staff.
- Present all acceptable referrals or follow up referral information to the appropriate administrators. Publish and distribute Pre-admission minutes and follow up with all requests for additional information.

Work with Administration to coordinate evaluations and admissions. Inform all appropriate staff via a memorandum of all scheduled evaluations, visits and/or admissions.

Participate in all appropriate staff meetings, pre-admission meetings, community visits and other meetings as directed by supervisor regarding admissions and discharges.

Complete and submit request for funding to appropriate Case Coordination Agency to secure funding for placement.
- Complete admission summary on all new admissions. Complete face sheet to be distributed at admission staffing to all appropriate staff and locations.
- Complete all paperwork for the Department of Human Services, Office of Public Aid and Office of Social Security Administration following admission staffing.

Secure a Release of Information for all discharges from Milestone, Inc.

Copy and mail all discharge information to the appropriate Case Coordination Agency. Follow up with status of referrals weekly. Inform supervisor of status on Monday of each week.

Complete Discharge Summary for all discharges from agency (move, death, discharge etc.).

Complete all discharge information to the Department of Human Services, Office of Public Aid, Office of Social Security Administration and Division of Developmental Disabilities (if applicable).

In compliance with the Milestone philosophy and policies toward Human Rights, immediately intervenes on behalf of the individual(s) served and reports to the appropriate agency (OIG, IDPH) and/or Supervisor.

Other Duties as assigned to ensure the safe, efficient operation of the agency.
QUALIFICATIONS:

Minimum of a BS or BA in Social Work, Developmental Disabilities, Education, Psychology, or related field.

Good communication skills.
-
Valid Drivers license.
-
Ability to work well with families of persons with developmental disabilities and community providers/agencies.
