- JOB SUMMARY
- Assembles, and reviews for completeness, medical records electronic and paper, of patients discharged from the hospital, and compiles data from medical records for periodic statistical reports.
- DUTIES AND RESPONSIBLITIES
- Audit patient records utilizing the Electronic Health Record and Paper Record when applicable
- Prep, Scan, Index, and QA additional documentation to electronic chart, verifying correct patient identifiers and indexing of correct name/placement for document.
- Verify QA of all scanned documents to system.
- Assembly of patient records in appropriate chart order and apply chart deficiencies and assign chart deficiencies to appropriate physician or provider for completion. (Utilizing the Electronic Health Record when applicable
- Verification of complete medical record with accurate patient identifiers and address incorrect identifiers as needed. (Utilizing the Electronic Health Record when applicable)
- Filing of complete medical records in accordance with department policies.
- Process requests of patient information and logging in request log as appropriate maintaining HIPAA compliance. (Utilizing the Electronic Health Record when applicable)
- Answers phone in a polite manner and communicate information to the appropriate person maintaining HIPAA compliance
- Prepares reports for statistical data reportable to various agencies maintaining HIPAA compliance.
- Knowledge and comprehension of the use of various Microsoft programs such as Word, Excel, Access, Outlook Express and PowerPoint
- Greet patients, visitors, co-workers and provide assistance as needed in a professional respectful manner.
- Maintain confidentiality of patient information at all times
- Maintain office equipment and supplies and prepare supply order as needed
- Attend routine and mandatory meetings and training
QUALIFICATIONS, EDUCATION, TRAINING AND WORK EXPERIENCE
High School Diploma or equivalent
Additional Languages preferred
At least (2) years of previous clerical work
Knowledge of medical terminology
