Position Overview

Our facility includes:
- 1 Vent/Trach unit
- 2 Short-Term Care units
- 5 Long-Term Care units
- Approximately 175 residents total

Key Responsibilities include, but are not limited to:
- Provide direct nursing care and medication administration in accordance with physician orders and care plans.
- Monitor resident conditions and promptly report changes to physicians and the interdisciplinary care team.
- Perform skilled nursing procedures, including wound care, tube feedings, catheter care, and specimen collection.
- Accurately document assessments, treatments, medications, and resident responses.
- Respond to resident needs, emergencies, and changes in condition during the NOC shift.
- Collaborate with CNAs, therapists, physicians, and other team members to ensure high-quality resident care.
- Ensure compliance with state regulations, facility policies, and infection control standards.
Requirements
- Active LPN or RN license in the state of Utah (REQUIRED prior to employment)
- Ability to walk extensively throughout a large facility and perform moderate lifting and physical tasks
- Strong teamwork, communication, and patient care skills
- Point Click Care skills
- All applicants must pass a background check and employment verification check
Pay: $30.00 - $42.00 per hour
