as a Community Connector and play a vital role in engaging, educating, and assisting individuals in accessing essential programs and services within UCAP and the broader community!
WHAT YOU'LL DO:
- Engage & Educate: Conduct outreach, presentations, and events to promote UCAP programs.
- Facilitate Access: Guide individuals through screening and enrollment for services.
- Champion Health Equity: Strengthen partnerships with healthcare and public services to support RIM communities.
- Develop Partnerships: Collaborate with BIPOC organizations for culturally responsive outreach.
- Support Participants: Help individuals navigate services, understand rights, and create action plans.
- Promote Collaboration: Build a strong referral network with community groups, schools, and
businesses.

EDUCATION AND EXPERIENCE:
- Minimum: Health Services Worker Certification OR a total of five (5) combined years of
education + relevant experience
- Preferred: Bachelor’s degree in Human Services (or related field) with two (2) years of human
services experience
