What You’ll Do
- Perform in-home PT evaluations and create care plans
- Help patients regain mobility and independence
- Coordinate care with physicians and care teams
- Supervise PTAs as applicable
- Track and report patient progress
What We’re Looking For
- Active Physical Therapist license
- BLS/CPR
- Valid Driver’s License
- Home health or high-acuity experience preferred
- Strong communication, organization, and autonomy
