Schedule: Full-Time
Salary: $65,000–$83,000 annually
Rehabilitation and Nursing Center is seeking a motivated, detail-oriented, and compassionate Admissions Coordinator to join our team. In this vital role, you will serve as the first point of contact for residents and families, ensuring a smooth admissions process while fostering strong relationships with referral sources and the community.
Identify and monitor operational areas that impact admissions; report findings and assist with implementing improvements
Support marketing and outreach initiatives by attending community events and developing referral relationships
Educate families and guests on facility policies, visitor guidelines, and resident care procedures
Coordinate admissions with hospitals and referral sources to ensure timely and efficient transitions
Build and maintain relationships with hospitals, physicians, and community partners
Verify financial and insurance information prior to admission and communicate details to the interdisciplinary team
Maintain current knowledge of Medicare, Medicaid, and Managed Care processes
Track, analyze, and report monthly admissions data and trends
Minimum of 2 years of admissions experience in a Skilled Nursing Facility
Strong understanding of Medicare, Medicaid, and insurance processes (including PDPM, HMO guidelines, and RUGS II & III)
Experience with EMR systems
Knowledge of CMS guidelines and Department of Health (DOH) regulations
Excellent communication, organizational, and interpersonal skills
