About the Role:
The Licensed Practical Nurse plays an essential role in delivering high-quality nursing care within an assisted living facility setting. This position involves providing direct patient care, including wound management, pain control, and performing venipuncture, ensuring that residents receive compassionate and effective treatment. The Licensed Practical Nurse will also be responsible for educating patients and their families about health conditions and care plans, promoting understanding and adherence to medical advice. Effective communication in English, both written and verbal, is essential to document patient progress accurately and collaborate with interdisciplinary healthcare teams. Ultimately, the Licensed Practical Nurse contributes to enhancing the quality of life for residents by maintaining a safe, supportive, and therapeutic environment in compliance with healthcare regulations and standards.
Minimum Qualifications:

Valid and current Licensed Practical Nurse (LPN) license in the United States.

Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare teams.

Demonstrated experience in wound care and pain management.

Ability to perform venipuncture safely and accurately.

Experience working in an assisted living facility or similar healthcare environment.
Preferred Qualifications:

Certification in wound care or pain management.

Additional training or certification in geriatric nursing or long-term care.

Experience with electronic health record (EHR) systems.

Strong patient education and counseling skills.

CPR and Basic Life Support (BLS) certification.
Responsibilities:

Provide comprehensive nursing care to residents, including wound care management and pain assessment.

Perform venipuncture and other clinical procedures as required to support diagnostic and treatment processes.

Educate patients and their families on health maintenance, medication administration, and post-care instructions.

Maintain accurate and timely documentation of patient care activities, observations, and clinical notes in English.

Collaborate with healthcare professionals to develop and implement individualized care plans within the assisted living facility.

Monitor patient conditions and report any changes or concerns promptly to the supervising nurse or physician.
