Position Overview
Seeking an experienced RN Case Manager to coordinate patients throughout acute-care hospitalization and manage discharge planning, transitions, utilization management, and quality screenings.
Key Responsibilities
- Coordinate patients throughout the acute-care hospitalization.
- Manage all aspects of the discharge planning and transition process.
- Perform utilization management and quality screenings.
- Coordinate appropriate discharge placements, including rehabilitation facilities.
- Support care planning for subacute patients.
- Collaborate with physicians, nursing staff, patients, families, and other healthcare professionals to ensure effective transitions of care.
Requirements
- Active RN license required
- BSN required
- Minimum 2 years of acute inpatient hospital experience
- Inpatient Case Management experience required
- Strong experience with acute inpatient discharge planning and placement
- Experience with subacute patients required
- Case Management / Managed Care experience
- CCMC preferred
- Hospital experience required
Schedule & Additional Details
- Monday–Friday, 8:00 AM–4:00 PM
- Rotating weekends: Every 4th or 5th weekend
- One summer and one winter holiday required
- Floating may be required as needed
- Guaranteed hours available
