Job Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this role, you will provide high-quality nursing care to patients in their homes or hospice facilities, ensuring comfort and dignity during end-of-life care. You will work closely with patients, families, and interdisciplinary teams to deliver comprehensive care tailored to individual needs.
Duties
- Conduct thorough assessments of patients’ health status, including vital signs and diagnostic evaluations.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, including catheter care and ventilatory support when necessary.
- Monitor patients for changes in condition and respond appropriately to emergencies.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life decisions.
- Maintain accurate medical documentation of patient care activities and progress.
- Assist with case management tasks, coordinating services and resources for patients as needed.
- Lift and transfer patients safely while adhering to best practices for patient handling.
Requirements
- Valid nursing license (RN or LPN) in the state of practice.
- Experience in nursing home or ICU settings preferred.
- Proficiency in assessing vital signs and performing diagnostic evaluations.
- Strong skills in case management and medical documentation.
- Ability to lift patients safely using appropriate techniques.
- Excellent communication skills with a focus on empathy and patient-centered care.
- Knowledge of hospice philosophy and palliative care principles is a plus.

Pay: $200.00 - $359.00 per day
