Overview
Join our dedicated healthcare team as a Home Care Physical Therapy Assistant (PTA), where you will play a vital role in delivering exceptional care to our patients. In this position, you will assist licensed physical therapists in implementing personalized treatment plans, supporting patient recovery, and promoting mobility and independence across diverse patient populations. Your expertise in manual therapy, therapeutic exercises, and clinical documentation will directly impact patients' quality of life, fostering a compassionate and energetic environment focused on healing and progress.
Duties
- Assist and support licensed physical therapists in developing and executing individualized care plans tailored to each patient’s needs, including geriatrics, pediatrics, orthopedics, and post-acute care.
- Implement physical therapy treatments.
- Operate medical equipment safely and effectively while adhering to clinical standards for documentation and patient safety.
- Document all treatment activities accurately within electronic health records , ensuring compliance with healthcare regulations and clinical documentation standards.
- Collaborate closely with healthcare professionals—including physicians, nurses, and other therapists—to coordinate care plans and optimize patient recovery strategies.
- Engage with patients through effective communication, addressing concerns, explaining procedures clearly, and fostering a positive therapeutic relationship during home visits or outpatient sessions.
Skills
- Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and medical terminology to inform effective treatment implementation.
- Proficiency in clinical assessment documentation and medical documentation practices to ensure accurate record-keeping.
- Experience with outpatient clinics, skilled nursing facilities, inpatient settings, or home health environments is highly desirable.
- Skilled in using medical equipment operation relevant to physical therapy interventions including therapeutic modalities and manual therapy tools.
- Excellent caregiver communication skills to educate patients and family members about treatment goals and care plans.
- Ability to work independently in home health settings while maintaining high standards of patient care and safety.
- Familiarity with discharge planning processes and managing care transitions across different healthcare environments.
