We are seeking a compassionate, driven, and experienced Certified Dementia Practitioner (CDP) to serve as a dedicated advocate and resource for our dementia care communities.
In this impactful role, you will lead initiatives that enhance quality of life, empower caregivers, educate staff, and help create innovative dementia-focused programs across our communities, including Butler, Rosewood, and Seasons. This position is ideal for a self-motivated professional who thrives on building relationships, championing resident-centered care, and driving positive outcomes.
As our Dementia Care Advocate & Program Specialist, you will:
Serve as a trusted expert and advocate for residents living with dementia and their families
Develop, implement, and continuously improve dementia-specific programs, activities, and best practices
Provide education, coaching, and support to clinical and non-clinical staff
Collaborate with interdisciplinary teams to enhance resident engagement, well-being, and quality of care
Partner with families to provide guidance, resources, and ongoing support throughout their loved one's journey
Build strong relationships with healthcare providers, referral sources, and community partners
Assist with outreach, referral development, marketing initiatives, and conversion efforts to support community growth
Promote person-centered approaches that preserve dignity, independence, and meaningful connections
Required Qualifications
Current Certified Dementia Practitioner (CDP) certification
Reliable transportation and ability to travel between assigned communities
Strong communication, presentation, and relationship-building skills
Self-starter with excellent organizational and problem-solving abilities
Passion for dementia care and resident advocacy
Ability to work independently while collaborating effectively with interdisciplinary teams
Experience supporting referral generation, marketing, community outreach, or occupancy growth initiatives
Previous experience working directly with individuals living with dementia
Formal education or specialized training in dementia care, memory care, gerontology, or related fields
Experience developing, implementing, and evaluating programs and educational initiatives
Background in senior living, healthcare, home health, hospice, or memory care settings
