We’re seeking a detail-oriented and collaborative MDS Coordinator to lead our resident assessment and care planning processes. If you’re a licensed RN with a passion for accuracy and interdisciplinary teamwork, we’d love to hear from you!
Job Duties:
Oversee and schedule timely completion of MDS assessments
Review and certify documentation for accuracy and compliance
Lead care planning meetings and conduct resident interviews
Enter and transmit assessment data per regulatory requirements
Analyze Quality Indicator Reports and track resident changes
Educate staff on MDS documentation and compliance standards
Collaborate with the Director of Nursing on assessment-related needs
Qualifications:
Graduate of an accredited nursing program
Current RN license
Strong communication, organizational, and analytical skills
Experience with electronic medical records and regulatory compliance
Ability to lift up to 25 lbs and work in a clinical environment
