Job Summary:
We are seeking a compassionate and experienced RN to coordinate and manage care for patients receiving hospice services. The Hospice Case Manager works closely with physicians, nurses, social workers, chaplains and families to ensure patients receive high-quality end-of-life care focused on comfort, dignity, and quality of life. Monday-Friday 8am-5pm with on-call rotations.
Training available for candidates who have experience in Long Term Care or ER environments.
Key Responsibilities:
- Assess patient needs and develop individualized hospice care plans
- Coordinate care among the interdisciplinary hospice team and attend IDG meetings
- Conduct regular patient visits and monitor health status
- Manage pain and symptom control in collaboration with physicians
- Educate patients and families about hospice services and care plans
- Maintain accurate documentation and medical records
- Ensure compliance with hospice regulations and policies
- Provide emotional support and guidance to patients and families
- Performs supervisory visits for hospice aides and LPNs
Qualifications:
- Active Registered Nurse (RN) license (MA state required)
- Minimum 1–2 years of clinical nursing experience
- Hospice or palliative care experience preferred
- Strong communication, organization, and care coordination skills
- Ability to work independently and travel to patient homes, long term care nursing facilities and assisted living facilities in the Framingham - 495 area

Pay: $85,000.00 - $97,000.00 per year
