Job Summary

Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe for any changes in health or behavior
- Administer medications accurately following care plans and HIPAA regulations to ensure privacy and safety
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Use specialized equipment such as Hoyer lifts or wheelchairs to safely transfer clients with mobility challenges
- Maintain detailed documentation of patient care, progress notes, and incident reports in compliance with healthcare standards
- Support clients with dementia, Alzheimer’s, or memory care needs through engaging activities and consistent routines
- Perform light housekeeping tasks such as cleaning, laundry, and organizing to promote a safe living environment
- Collaborate with healthcare professionals to develop and implement personalized care plans
- Provide emotional support and companionship to foster social engagement and mental well-being
Skills
- Proven experience in home health, assisted living, nursing home, or group home settings with a focus on senior or disability care
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Strong understanding of HIPAA regulations to ensure client confidentiality
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Excellent communication skills for working with clients with developmental disabilities or autism spectrum disorder
- Experience working with individuals experiencing dementia or memory loss in a compassionate manner
- Capable of following detailed care plans while adapting to changing client needs
- Skilled in cleaning, organizing, and maintaining a safe home environment for clients
- Familiarity with hospice care principles and end-of-life support services is a plus
