CPC Integrated Health is seeking a dedicated and motivated Behavioral Health Clinician I (LSW/LAC) to join our innovative Mental Health Diversion Court Pilot Program. Position Summary:
As a Behavioral Health Clinician I for the Mental Health Diversion Court Program, you will play a vital role in helping individuals with serious mental illness successfully engage in treatment while navigating the court system. You will work closely with attorneys, court personnel, law enforcement, community providers, and CPC Integrated Health programs to coordinate services that promote recovery, stability, and reduced involvement in the criminal justice system.This position is primarily community-based and includes regular travel throughout Monmouth County to courts, community providers, and client locations.
Minimum Qualifications Required:
- Master's degree in Social Work, Counseling, or a related behavioral health field.
- Licensed Social Worker (LSW) or Licensed Associate Counselor (LAC) in the State of New Jersey required.
- Experience working with individuals with serious mental illness, co-occurring disorders, or justice-involved populations preferred.
- Strong assessment, case management, crisis intervention, and care coordination skills.
- Ability to build collaborative relationships with courts, attorneys, law enforcement, and community partners.
- Excellent written and verbal communication skills.
- Valid New Jersey driver's license and reliable transportation required.
Responsibilities:
- Conduct comprehensive, culturally responsive biopsychosocial assessments and determine eligibility for the Mental Health Diversion Court Program.
- Provide ongoing case management and care coordination for individuals enrolled in the program.
- Develop individualized service plans and connect clients with appropriate behavioral health, medical, housing, substance use, vocational, and community resources.
- Coordinate services with attorneys, law enforcement, court personnel, hospitals, and community providers to support participant success.
- Serve as the primary liaison between the court system and CPC Integrated Health programs.
- Monitor participant engagement, treatment compliance, and progress toward recovery goals.
- Advocate for participants by addressing barriers to treatment, housing, employment, transportation, and other social determinants of health.
- Maintain timely, accurate, and comprehensive clinical documentation in accordance with agency, funding, and regulatory requirements.
- Attend court hearings, multidisciplinary team meetings, and case conferences as required.
- Adhere to all agency policies and procedures, including attendance at required trainings and meetings.

The position requires prolonged sitting, some bending and stretching. Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
