Responsibilities:
Provides skilled SLP services and interventions in accordance with regulations and need
Evaluates, sets a course of action
Implements and modifies the patient’s plan of treatment
Swallow studies and works with patients during meal times
Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate.
Provide clinical support and education to patients, family members, and caregivers.
Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
Attend required meetings as designated by the Director of Rehab.
Requirements:
SNF/Long term care experience preferred
Current knowledge of treatment practices
Knowledge of Net Health Electronic Documentation System preferred
