Blended Case Manager (BCM) Position
Location: 4641 Roosevelt Blvd., Philadelphia, PA
Work Schedule: Full-time position, Monday through Friday, 9:00 a.m. to 5:00 p.m.
Position Overview:
Blended Case Managers (BCMs) spend approximately 75% of their work time in the community meeting with program participants. BCMs are required to provide a minimum of 3.5 billable service hours per day and conduct at least one face-to-face visit per month with each assigned participant.
Visits may take place in participants’ homes, psychiatric or medical hospitals, residential facilities, or other community-based locations.
The BCM is responsible for assessing participants’ needs, developing and coordinating service plans, linking participants to community resources, advocating on their behalf, monitoring progress, and providing ongoing support to help individuals achieve their recovery goals.
Qualifications:
- Bachelor’s degree in a mental health-related field.
- Minimum of one year of related experience.
- Valid driver’s license with a clean driving record.
Salary & Benefits:
- Starting salary: $40,131 annually ($20.58/hour)
- Four (4) weeks of paid vacation during the first year
- Twelve (12) paid sick days
