We are looking for a dedicated and experienced In-Home High Acuity Nurse to provide exceptional care to patients in a home setting. The ideal candidate will have a strong background in critical care, with experience in the Intensive Care Unit (ICU) and Neonatal Care Unit (NCU).
Key Responsibilities:
- Deliver high-quality nursing care to patients with complex medical conditions in their home environment.
- Manage care for the patient who has Tracheostomy.
- Conduct thorough patient assessments and develop individualized care plans in collaboration with the healthcare team and the patient's family.
- Monitor vital signs and respond to changes in the patient's condition promptly.
- Educate patients and their families about care plans, medications, and lifestyle adjustments.
Qualifications:
- RN license with proven experience in critical care, ICU, and NCU.
- Expertise in managing patients with oxygen support and tracheotomy .
- Strong critical thinking and problem-solving skills.
- Excellent communication and interpersonal skills, particularly in a home care setting.
Schedule:
- Saturday, Sunday, Monday
