BreakThru Services is seeking qualified On-Call Guardianship Coverage Specialists to provide after-hours, weekend, and emergency coverage for individuals under BreakThru's guardianship. On-call staff are activated when the primary Guardianship Coordinator is unavailable due to planned leave, illness, or emergency circumstances. Coverage specialists must be available to respond to urgent healthcare decisions, emergency placements, crisis situations, and other time-sensitive guardian responsibilities on behalf of individuals with intellectual and developmental disabilities. This is a per diem role ideal for experienced social workers, counselors, or human services professionals seeking flexible supplemental work.
Key Responsibilities:
Respond to after-hours, weekend, or holiday calls from hospitals, residential providers, courts, or emergency services regarding individuals under BreakThru's guardianship
Make time-sensitive guardian decisions regarding emergency healthcare, safety, and welfare on behalf of incapacitated individuals in accordance with the individual's care plan and the Program Director's guidance
Conduct emergency client visits when required to assess safety, health status, or welfare of a ward in crisis
Communicate with the Program Director and primary Guardianship Coordinator on all coverage activations and document all actions taken during the coverage period
Coordinate with hospitals, emergency rooms, Adult Protective Services, law enforcement, and residential providers as needed during coverage periods
Maintain detailed written documentation of all coverage activities, decisions made, and contacts during each coverage period
Maintain availability according to the agreed upon on-call schedule and respond to coverage activations within the required timeframe
Minimum Qualifications (Required):
Bachelor's degree minimum in social work, psychology, counseling, human services, or a related field; Master's degree or LCSW/LPC licensure strongly preferred
Minimum of 2 years of experience working with individuals with IDD, serious mental illness, or complex behavioral health needs
Strong crisis intervention skills and experience making time-sensitive clinical and welfare decisions
Completion of Virginia DARS Guardian Training (or willingness to complete prior to first coverage activation)
Availability to respond to coverage requests within 60 minutes during designated on-call periods
Valid Virginia driver's license and reliable personal transportation
Ability to pass a comprehensive background check including criminal history and adult abuse registry clearance
Qualifications (Preferred):
LCSW, LPC, or equivalent QMHP-level licensure
Prior experience in guardian, conservator, crisis stabilization, or emergency response roles
Familiarity with Virginia DD Medicaid waiver services, community resources, and DBHDS regulations
Experience working with DBHDS-licensed residential or day program providers
