Job Summary
A Plus Home Health Care is a Medicare-certified agency, and we are seeking a dedicated and compassionate Registered Nurse to join our healthcare team as DPCS. A Plus Home Health is growing, and we're looking for a passionate candidate with a strong background in patient care and experience in various nursing specialties to be our DPCS. This role is critical in providing high-quality one-on-one skilled nursing care to patients across different settings, and supervise the nursing staff, for example, patients' homes and various facilities. The Registered Nurse will assess patient needs, develop care plans, OASIS, QA, oversee nursing staff and collaborate with family members or other healthcare professionals to ensure optimal patient outcomes.
Duties Include:
- Conduct thorough assessments of patient's health status and needs.
- OASIS
- QA
- Develop, implement, and evaluate individualized nursing care plans.
- Administer medications and treatments as prescribed.
- Assist in discharge planning to ensure a smooth transition for patients from A Plus Home Health.
- Perform triage duties in emergency situations, prioritizing patient care based on urgency.
- Monitor patients' progress and respond to changes in their condition promptly.
- Educate patients and their families about health management and treatment options.
- Maintain accurate documentation of patient care activities and outcomes.
Qualification Requirements:
- Current Registered Nurse (RN) license in the state of practice.
- Prior Home Health experience is preferred
- Ability to work independently and demonstrate critical thinking skills if needed
- Strong knowledge of physiology and nursing principles.
- Familiarity with discharge planning processes is A Plus ;)
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team.
- Compassionate demeanor with a strong commitment to patient-centered care
- Be a licensed driver with an insured automobile in good working order

Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in inpatient or outpatient settings is preferred.
- Familiarity with medical records management, discharge planning, and case management processes is a plus.
- Experience in travel nursing or working with diverse patient populations is advantageous.
- Strong skills in diagnostic evaluation and specimen collection processing are desirable.
